Elopement, Notice of Incident

While outside during activity time, 3 residents told staff they were going to foot race to see who was the fastest. The boys didn’t stop running when they got to the “finish line”.

Code Orange was called and staff went after the boys. Police were also contacted. Multiple staff and department heads went out and started searching for the boys.

The boys were located 3/4 mile down the road by the power lines. The boys were gone for 30 minutes. The boys were returned to Millcreek and placed on unit restriction and elopement precautions.

A resident was sent to Dallas County Medical Center for a chest x-ray due to cough, congestion, and fever. Flamingo hall has been quarantined due to multiple respiratory illnesses.

A resident went to the Dallas County Medical Center Emergency Room due to a high fever. The resident returned to Millcreek that afternoon. Several residents in the same house are being x-rayed for respiratory illness. The House has been quarantined.

A resident was sent to the Dallas County Medical Center ER due to high fever, elevated pulse ox, and heart rate. She was admitted to the hospital and returned to the facility on 8/31/24.

Several residents in the same house are being x-rayed for respiratory illness. The house was quarantined.

Officers were dispatched to assist with locating a missing juvenile. Officers deployed an arial drone and located the subject. The resident was taken into custody and returned to Millcreek with no injuries.

A resident was in school and ran out of the side doors. Supervisors followed the resident but lost sight of her at 13:40 after she turned at the stop sign and went into the woods. Police were contacted and came to assist with the search using a drone. The resident was located at 14:58 in woods and returned to unit. She has been placed on unit restriction.

Resident stated she brought a vape into the facility with her, and that a staff member found her vape charger and told her to give it to them so she wouldn’t get in trouble. Resident states the staff member keeps the vape in her car and will bring it in during her shift for resident to use. A report was made to AR Child Abuse Hotline and accepted. Staff member was placed on administrative leave.

Licensing Specialist: Clayton DeBoer

Emily Burris of Millcreek reported that the resident stated it was the first weekend at the facility that the staff member let her vape, and that video was reviewed with nothing found. The resident reported this occurred in her room. Staff member works 3rd shift and roommate has never seen this occur. The resident stated she brought the vape into Millcreek in her bra. Pat downs are conducted upon entry but not around the bra area. Witness statements were reviewed, with alleged staff member and resident’s peers stating that at no time did the staff member provide a vape to resident.

11/21/24 – CACD case unfounded. This complaint has been unfounded by Licensing. As a precautionary measure, the staff member will be retrained on staff/client boundaries.

Notice of Incident, Peer Altercation

A resident was involved in a physical altercation with several other girls. During the altercation, the resident sustained an injury and was taken to Dallas County Medical Center for treatment.

Licensing Specialist: Clayton DeBoer

Licensing visited the facility and reviewed video of the incident. Ratio in the nurses’ station begins 2:3 and ends in 7:6 after the altercation breaks out. Several residents are seen attacking the resident. Staff are present and immediately intervene. Additional staff are seen assisting in breaking up and de-escalating the altercation.

During a behavior, a resident attempted to kick staff. The staff member grabbed the resident’s leg and pulled him out of the chair to the floor. Staff walked out immediately following the incident and has been terminated.

LS DeBoer visited the facility in response to the incident. Video of the incident was reviewed and showed staff aggressively grabbing the resident’s leg and pulling him out of the chair until he hits the floor.

Facility was cited 109.1.g: Unprofessional conduct in the practice of child welfare activities and 905.4.g: The following actions shall not be used, including as discipline: physical injury or threat of bodily harm.

Facility was also cited 110.9.a: Any owner, operator, or employee in a child welfare agency whall immediately notify the Child Abuse Hotline if they have reasonable cause to suspect that a child has been subjected to child maltreatment due to late reporting. A unit-wide training will be held on mandatory reporting and sign-in sheets will be provided to Licensing.

Visit Compliance Report

Licensing Specialist: Clayton DeBoer

A visit to the facility was conducted in order to review fire and severe weather drill logs. Fire logs were documented per licensing standards and completed every month until recently. No severe weather log could be located. The Maintenance Supervisor stated that the severe weather logs were not available.

Facility was cited 914.9 : A record of drills shall be maintained, showing date and time of day of the drill, number of participants, and length of time required to reach safety.

Risk Manager Burris stated that severe weather drills have been completed. Severe weather logs will be located and provided to licensing by 8/9/24.

A Complaint Investigation survey was conducted on July 31, 2024. No deficiencies were cited during the survey. Complaint #AR00033837 was in compliance.

Notice of Incident, Peer Altercation

A resident and a peer got into a verbal altercation, then the peer hit the resident in the nose. The resident was sent to Dallas County Medical Center for an x-ray. X-ray and CT scan showed a nondisplaced fracture of the right nasal bone.

The resident and her peer were moved to separate units.

The facility was in compliance with §483.475 – life safety from fire.

A recertification survey was conducted at this facility. This survey found the facility had deficiencies requiring correction/substantial correction.

Deficiencies found: W104 – Buildings and Grounds, W369 – Drug Administration Policy, W390 – Drug Labeling Policy, W454 – Infection Control.

Facility submitted a Plan of Correction for deficiencies noted.

Police Report

Assault, Police Report

Officers were dispatched to Millcreek regarding an assault by a staff member. Upon arrival, the facility CEO advised officers of the situation and that the staff member had resigned. Officers took photos of the injuries and advised the resident’s guardian to come by the police department so an interview could be conducted.

After an intervention, a resident reported that he was pinched on his chest and scratched on his shoulder. A nursing assessment confirmed a .5″ x 3″ scratch on his right shoulder and a quarter-sized bruise on his chest. The accused staff member chose to resign immediately after the report.

Facility visited in response to the incident. Camera footage was reviewed and showed staff pinching the resident.

Resident was sent to Dallas County Medical Center’s ER for outpatient X-rays after reporting ongoing pain and swelling in her left ankle for about two weeks. She had not previously sought assessment for this issue. The nurse’s initial impression was that it could be fatty tissue with possible fluid retention.

The X-ray report showed no fractures present. The resident was then released to the facility without any restriction or need for treatment.

Resident was upset at a peer and kicked the door. Afterward, he complained of knee pain and swelling. He was taken to Dallas County Medical Center and received an x-ray of his knee.

Diagnosis: Contusion of the knee. He returned to Millcreek the same day.

Resident was in an altercation with a peer when the peer hit him in the face. Staff immediately separated the two residents. The next morning, the resident complained of jaw pain. He was sent to Dallas County Medical Center for an x-ray of his jaw. No fractures were noted.

Accidental Injury, Notice of Incident

Resident complained of knee pain and stated that he hurt his knee playing on the waterslides yesterday. Resident was taken to Dallas County Medical Center and diagnosed with a left knee sprain. He was instructed to rest and elevate for 3 days. He then returned to Millcreek.

Buildings & Grounds conducted for Flamingo and Zebra Hall.

Flamingo staff/client ratio: 4:16. Sleeping areas furnished, clean, safe, and in good repair. Bathrooms are clean and sanitary. Client rights and emergency escape diagrams observed.

Zebra staff/client ratio: 2:16. 2 present staff were asked where 3rd staff member was as Zebra is staffed for 3 Behavioral Health Associates. 3rd staff member was reported to be outside of Zebra’s door making a phone call. Outside area was observed and 3rd staff member was not seen. Radio call was made, and later 3rd staff member was seen entering Zebra Hall. Facility cited 907.3.

Sleeping areas furnished, clean, safe, and in good repair. Bathrooms clean and sanitary. MARs checked for residents. All initialed and up to date.

Buildings and Grounds were completed for Penguin Hall, Eagle Hall, and the outdoor area.

Outdoor picnic area staff/client ratio: 4:20. Grounds were clean and free of safety hazards.

Eagle Hall staff/client ratio: 4:11. Building was clean, safe, and in good repair. Bathrooms are clean and sanitary with functioning sinks, showers, and toilets.

Penguin Hall clients outside. Building was clean, safe, and in good repair. Bathrooms are clean and sanitary with functioning sinks, showers, and toilets.
