Escape from Juvenile Detention Facility, Second Degree Escape

Officers were dispatched at approximately 1309 hours in reference to a runaway Juvenile; Staff advised Juvenile had ran from the property 20 minutes prior. On 6/12/19 at approximately 1940 hours, Juvenile was located.

Caller reported suspicious acitivity on side of interstate; Caller noticed several cars and a van parked on the side of the road and advised 5 people chased a Juvenile into the woods. After contacting facility, Staff advised that they were trying to detain the runaway. Staff also informed Officers that they had found the 2 missing Juveniles that went missing on 6/9/2019 (above).

Staff reported 2 Juveniles had ran away from facility. Officers were unable to locate Juveniles at the time. A Deputy had picked up a Juvenile while patrolling, and upon returning Juvenile to facility the Staff stated that this Juvenile was a different runaway, and that the 2 Juveniles originally reported were still missing.

Police contacted Staff at facility to check if 2 Juveniles that had ran away had returned to facility. Staff stated the 2 Juveniles were located the same day they had ran away. Staff stated all Residents were accounted for at that time.

    • N 196 MEDICAL TREATMENT FOR INJURIES – Facility failed to ensure prompt medical treatment was obtained to prevent further potential complications for 1 client who sustained an injury during an ESI.

A revisit was conducted on 7/23/19 for all deficiencies cited on 6/4/19. All deficiencies have been corrected, and no new noncompliance was found

    • Zebra Hall – Room 304 has cracked storage bin with sharp edge.

    • Eagle Hall – Room 602 had pull ties and room 606 had pull ties.
    • Tiger Hall – Room 501,503,505,506,508,509, and 510 had pull ties and cords.
    • Gym – Fire Extinguisher cover broken with exposed metal hanging piece accessible.
    • Magnolia – Has walls that need repair and light not working in bedroom.
    • Rock Hill – Water leak in back bedroom closet floor.
    • Pine Ridge – Pull ties in bedrooms 3 and 7.
    • Pebble Creek – Pull ties in bedroom 5. floor baseboards in bedroom 6 need repair.
    • Haley – Pull ties left in bedroom.
    • Barn – Pet medications stored in closet left unlocked. Corrective Action required due to previous client injury for ingesting pet medications.

    • Discussed that a building sweep of PRTF program may be needed to ensure that pull ties and cords and other hazardous materials are removed from bedrooms.
    • Discussed that agency needs to submit a Corrective Action Plan to address medications in barn being left unlocked after previous Corrective Action Plan was in place due to previous client injury.

Staff reported 3 missing Juveniles and advised there is a facility van trying to locate the Juveniles. All subjects were located.

Fleeing, Purpose of Causing Injury, Second Degree Escape, Third Degree Battery

Staff reported runaway Juvenile; Officer located Juvenile walking into gas station. Juvenile broke free from Officer and took off running; As another Officer was coming around the corner, Juvenile charged at him knocking him to the ground. The 2 Officers were advised by another Officer that he had the Juvenile in custody after he ran into a field. Officers transported Juvenile back to facility.

Staff stated 2 Juveniles had escaped from facility. Officers searched area with negative results. A short time later, Staff notified police that the Juveniles had been found.
