Capacity: 162 (111 Arkansas Medicaid PRTF beds, 51 Residential beds used for out-of-state resident PRTF placement or AR contract beds).

Contact with family (Calls and visit schedule): Family visits vary on a case-by-case basis. Phone Calls are allowed on 4 days of each week and are limited to 10 minutes unless otherwise specified.

Police Report

Criminal Trespass, Disorderly Conduct

Staff reported another Staff member yelled at supervisor and then was taken to CEOs office to calm down. Staff had been sent home the previous day due to stating “everyone here lies” in a staff meeting. Staff was upset since supervisor told Staff that they would call him to let him know that status of his job. Staff left & stated they wouldn’t return until asked by facility.

Staff reported 4 Residents ran away from facility. Staff reported shortly after that all 4 Residents were back at the facility.

  • Provider Reported incident converted to complaint on 9/11. Incident date 9/10
  • Resident stated staff grabbed them, put arm behind back with staff’s knee in his back and he could not breathe. Not the first time. Also, staff “placed his jaw and head on the bed really hard.” Staff denied. Small knot in center of forehead.
  • Staff placed on admin leave pending investigation.  
  • Last visit on 9/11/20 – items cited at visit are awaiting corrections.  1 founded maltreatment and 18 founded licensing complaints in the last year.

Abuse Complaint, PRLS Information

  • Resident reported staff slapped her in the face for tearing things off the wall. Resident then ran to the door and staff twisted her wrist, so she hit them. Another staff became involved and hit and was hit by the resident. Staff 2 also grabbed resident by the neck and choked her. Staff says only time touched resident was to escort her to her assigned area.
    • Resident had discoloration under both eyes.
  • Involved staff placed on admin leave pending investigation. Not marked whether accepted by hotline.

Staff reported 2 Residents ran away from facility. Officers were able to locate both Residents, with 1 Resident resisting arrest. Both were escorted back to facility.

Visit Compliance Report

  • 912.2 – Out of compliance – Sinks not working in Penguin or Eagle bathrooms. 4th visit in 2020 with sinks not working.
  • TA – 912.4, 911.6, 910.1– showers need deep cleaning to remove buildup (2nd mention in 2020) tissue dispensers need repaired or replaced. Paint chipping, missing tiles, broken window, hole in wall, possible rat droppings found in the pots and pan cabinet in the kitchen (willow house). Missing toilet seat, no lock on cabinet under sink with chemicals in boy’s ranch.
  • Several units out of ratio, including Flamingo 3:19, Zebra 3:20, Eagle 2:18, Kangaroo 2:20

Abuse Complaint, PRLS Information

  • Incident on 08/23, PRI converted to complaint on 08/26. Accepted by CACD: Carolyn Gamble.
  • Resident reported to therapist was grabbed and thrown into bathroom door by 2 staff. Resident 2 reported being drug, thrown, and punched by Staff 1. Resident 3 reported he was slammed on his bed by Staff 2. Resident 4 reported he was grabbed on the neck and arm and slammed into bed and was scared of Staff 1.
    • Only Resident 1 assessed for injuries. Bruise, scab, and scratch on arm
  • S-1 not working direct and unsupervised with children. S-2 placed on admin leave pending investigation findings.
  • Licensing Complaint Notice. In response to report received 08/26/20 corrective measures required: 10/16/20 – S1 and S2 were retrained on agencies polices. (pg. 20)
  • 1 founded maltreatment complaint in last year. 18 founded licensing complaints in last year.

Abuse Complaint, PRLS Information

  • On 9/3/20 patient reported a staff member was giving 2 patients marijuana and they are smoking it on the playground. Patient’s drug tested, one positive, one inconclusive.
  • Staff has been terminated.
  • Licensing Complaint Determination Notice. Complaint received 9/3/20 unfounded. “There wasn’t substantial evidence found during the investigation to support the allegations.” (pg. 21)
    • Unfounded/staff terminated/positive drug screen.
  • Licensing Complaint Notice. In response to complaint received on 9/3/20 the following corrective measures are required: S1 and S2 completed retraining on the agency’s policies. (p 22)
    • PRLS said staff was terminated, giving illicit drugs to minors is not a retrainable incident, 2 staff are referenced here but only one staff everywhere else.
  • 6/26/20 last monitor visit, all cited items have been corrected. …favorable compliance…History – 1 founded maltreatment complaint in last year, 18 founded licensing complaints in last year.

Staff requested to report an allegation. Staff advised that Residents Guardian reported that the Resident had been slapped by another Staff member. Hotline was called, accepted allegation.

Staff reported a small bag of drugs (“leafy green substance”) was found at facility in the gym. Bag was turned over to police.

Staff requested to report an allegation. Staff advised that Resident stated to DHS via Facetime that another Staff member knocked water our of Residents hand and pushed Resident. Hotline was called, denied allegation.

Possible Staff Abuse Report

Staff requested to report an allegation. She advised that a Juvenile had informed his mother that a staff member had hit him. Child Abuse Hotline was advised and rejected the allegation.

Battery Third Degree, Recklessly Causes Injury

Officer was dispatched to facility for individual who refused to leave. Officer made contact with staff, who stated that another individual quit his job and the 2 got into an argument and individual pushed staff member. Officer advised staff of the affidavit process for 3rd degree battery. Staff had no marks and individual left prior to officers arrival.

Rape

Officer received a notice in the mail from investigator with Arkansas State Police Crimes Against Children in reference to a rape. Investigator stated that after the investigation so far, they don’t believe it happened. Investigator stated the victim did go to Arkansas Childrens Hospital for exam; the sexual assault kit is at Childrens Hospital. Allegation was determined to be unsubstantiated. Case was closed.

Nurse from facility reported that they had 2 Juveniles run away. Nurse reported approximately an hour later that both Juveniles had been located and were back at the facility.

Nurse from facility reported that Juvenile had ran away from facility about 30 minutes prior. Nurse called back a few minutes later to inform officer that Juvenile had been located.
