Fraudulent Use of a Credit Card, Other Reasons for Unauthorized Use of Card
Staff stated a Residents debit card was missing from the Business Office and several fraudulent charges were made with it. Staff stated new policies were in place to ensure the safe keeping of the patient’s money. Staff stated on 4/2/19 that they had a picture from video of Staff member taking the card. Staff stated they would file an affidavit; On 4/11/19 Officer issued Staff member involved a court date.

Staff reported at approximately 1757 hours a runaway Juvenile. After checking the area, Staff informed Officer that Juvenile had been located and was being brought back to facility.

Staff reported a runaway Juvenile at approximately 1356 hours. Staff advised police (at approximately 1643 hours) that they had found the missing Juvenile.

Staff stated a Resident had her credit card stolen; Bank informed them the credit card had been used 2/20 in the amount of $25. Staff stated the pin number had been written on the outside of the envelope the card had been in.

Officers were advised it was a false alarm; Staff reported Juvenile (at approximately 1930 hours) that had ran away “30-40 minutes ago.” At approximately 2117 hours, Staff advised they had found the Juvenile.

Staff reported 4 runaway Juveniles. Officers unable to locate. Later that day, Officer made contact with all 4 Juveniles and transported 2 Juveniles back to facility, while Staff transported other 2 Juveniles.

Staff advised an allegation had been made against an employee possibly hitting a Juvenile with some keys. Staff stated he was investigating the allegation but wanted to make a report. Officers later tried to make contact with staff using # they provided, no answer.

Caller (Resident) advised she needed the police to take her to the doctor, that she had swallowed a blade; ProMed responding to Millcreek. Staff stated Caller does this all the time.

Caller requested a welfare check on Juvenile at facility, Caller stated Juvenile had informed her that another Juvenile had choked and punched him. Officer made contact with Juvenile, and called Caller back to inform her everything was okay.
