Client was sitting on his knees in the floor playing a game. When he rose from the floor, his knee popped and began hurting. Client was taken to Dallas County Medical Center Emergency Department for an x-ray of his right knee. He was discharged back to Millcreek same day.
Diagnosis: Knee Sprain. Knee was wrapped and client was given crutches.

Client was sent to Dallas County Medical Center for a scheduled CT of head and US thyroid. Scans were ordered due to recent “fainting” spells. Patient reports that she often blacks out when she gets anxious

Complaint received: On 6/7/24, a client got mad and hit a staff member. The staff member stepped on the client’s feet to keep her in place. The client pushed the staff member away, but the staff member grabbed the client’s hair, pulling 6 braids out of her head. Staff didn’t follow proper protocol. They didn’t follow-up with a nurse or send an incident report to the client’s therapist. Staff at the facility are saying that the cameras were being worked on and were not working at the time of the altercation. Staff also changed the story, saying that the client pulled her own braids out of her head.

6/11/24 – Facility visited in response to complaint. Staff, Client, and Witnesses were interviewed. Nursing note related to the incident does not mention hair being pulled out. Witnesses interviewed stated that staff were “nice”.
LIcensing has determined that complaint is UNFOUNDED. As a precautionary measure, the alleged offending staff member received re-training.

Resident was sent to Dallas County Medical Center Emergency Department for evaluation due to having an altered mental status after an altercation with a peer. Resident was returned to Millcreek the same day, diagnosed with a mild concussion.
5/31/24 – Reviewed for licensing concerns. Email sent to facility to inquire if there is video. Received email that there is no video of the incident. Safety plan for resident sent to licensing.

Resident informed staff members that she got Ibuprofen out of a staff member’s bag and took it.
Resident was taken to Dallas County Medical Center ER for evaluation. She returned to Millcreek same day. Resident states that she took 3 handfuls of ibuprofen. Staff states that the bottle was almost empty and there were not 3 handfuls of pills in the bottle.

Visited facility in response to incident. Client showed no sign of overdose. Staff stated that she had only brought enough for personal use for the day.
Technical Assistance given for regulation 910.1 – The grounds of the facility shall be kept clean and free of safety hazards. All staff will receive training on personal medication storage by 6/30/24.

Resident put a cap off of a marker into her vagina. Nursing tried to talk her through removing it but was unsuccessful.
Resident was taken to Dallas County Medical Center Emergency Department. Patient was discharged back to Millcreek with no new orders.

Resident reported that she had a physical altercation with a staff member. Resident stated that she struck the staff member, and then the staff member hit her back with a closed fist. Afterward, the resident was placed in restraints. Several days later, she was placed in restraints a second time and was scratched.

5/30/24 – LS DeBoer visited the facility and reviewed video footage of the restraint on 5/22/24, which did not show staff doing anything harmful to the client.
Video was reviewed for the second restraint that occurred on 5/28/24. The alleged incident occurred in the client’s bedroom, which does not contain camera coverage. Various staff are seen in the hallway, entering and exiting the client’s room, including the alleged offending staff member. Incident reports for both incidents were reviewed.
This complaint has been UNFOUNDED by Licensing. As a precautionary measure, the alleged offending staff member will be retrained on restraint techniques.

Client was being disruptive. Staff were talking with him when two patients ran up to the client and kicked him in the face and side. Staff immediately separated and called nursing for an evaluation.
Client complained of rib pain and was taken to Dallas County Medical Center (DCMC) for evaluation. He was diagnosed with a rib fracture and sent home with no activity limitations.

Resident stated that he snuck up behind staff and staff turned around quickly, causing the keys on her lanyard to swing out and cut him under his eye. Deerfield building clean, safe, and in good repair except baseboard trim purposely removed from sleeping areas due to upcoming paint job. Client rights observed. Bathrooms clean and sanitary with functioning sinks, showers, and toilets. Grounds clean and free of safety hazards.
School clean, safe, and in good repair. Classroom ratios: 4:14, 4:10, 2:7, 5:16, and 5:15. Staff seen escorting clients across campus. Staff/client ratio 2:0. MARS checked. All initialed and up to date.

Client complained of right hip and leg pain. Client stated that it’s been hurting since before arrival at Millcreek, but it’s gotten worse. Client was taken to the Dallas County Medical Center on 4.30.2024, for x-rays of right hip and femur. No fractures or dislocations found.

Client was outside playing football in the field behind the barn and stepped on a nail. Client was taken to Dallas County Medical Center to receive a tetanus shot. There has been no recent construction in the area. Maintenance is running a magnet through the field to check for any other objects that might have washed up during the recent rainstorms.

Officer was dispatched to Millcreek in reference to a parent filing a complaint against a staff member. COO of Millcreek, Chris Butler, stated that the resident’s father informed him that he would be removing the resident from the facility. Day shift manager Mrs. Edwards, stated she was informed that a staff member had called the resident a “faggot” and a “nigger”.
Officer made contact with the resident’s mother who stated that the resident told her about the incident when she arrived for a visit. The resident stated that the staff member called him a “faggot” and a “nigger”, slammed him on the ground, and threw him across the room. Resident stated he tried to leave the room and call for help, but the staff member pulled him back inside the room. The resident’s mother stated she requested a statement be filed regarding the situation, but the statement had not yet been filed. Resident’s mother stated that when she did a full body audit on her child, she observed a blood spot under the skin on his right eye.
Officer made contact with accused staff member. He stated he did nothing wrong and was putting the resident in time out for acting up. Staff member said resident was screaming, cussing, and threatening to kill people.
Staff member was suspended pending an internal investigation into the incident.

Resident was having an on-campus visit with his mother on 4.13.24 and reported to his mother that a staff member called him a “faggot” and a “nigger”. Resident also stated that he was having an episode of physical aggression and that the staff member physically escorted him away from the situation and brought him to his room. The resident reports that when he arrived to his room, the staff member “threw him around.” The events allegedly happened on 4.12.2024. The mother reported these events to Millcreek staff and informed them that she would discharge the resident from the facility. She filed a police report with the Fordyce Police Department. Officers arrived on Campus and took statements from both the mother and the resident concerning the allegations. A Child Abuse Hotline report was made and accepted for investigation.

Facility visited 4/15/24. Staff member was placed on administrative leave pending investigation. There is no video footage of this incident. There are no documented incidents (4/12/24) of this nature in the client’s file. Resident was discharged from Millcreek.

Resident was sent to Dallas County Medical Center ER for assessment of a possible allergic reaction. Client was assessed at DCMC and given Benadryl before being discharged.
Facility stated the following “It was determined that she did have a minor allergic reaction. She did have a rash, but was found to be in no acute distress. Vitals all normal. She was admitted to ER at 1715 and discharged at 1745. The assumption was contact with bananas.”
