Notice of Incident, Peer Altercation

Medical emergency (Trip to er/Urgent care). After incident on 5/29 (See Incident Report), including a face to face assessment from nursing that concluded she was not concussed, client convinced her mother that she had a concussion. Pt has voiced multiple times to peers, staff, and mother that she feels like she has a concussion from altercation on Monday were she received head injury. Pt states she has felt “Nauseous at times” but no actual vomiting. APRN ordered client to be seen by the emergency department. Client returned from ACH ER with diagnosis of no concussion detected. Sent to ACH ER instead of urgent care for evaluation in case of the need for a cat scan, that procedure would only be available at the ER. Licensing narrative: 6/2/2023, Licensing Specialist received documentation for provider reported incident. 6/6/2023, Licensing Specialist reviewed provider reported incident for licensing concerns.

Battery, Fight Call

Police responded to call for an active fight in progress. Caller advised there were four female juveniles fighting in two different locations and staff were not able to get them under control. Staff advised two juveniles were in a minor altercation and one kicked a staff member in the groin when he attempted to physically separate them. The altercations involved juveniles tearing up each others items, throwing water and ice cubes, and pushing each other. All juveniles admitted to hitting someone, however, only 1 juvenile suffered from minor injuries and was cleared by the on staff nurse. Staff kicked in groin wished to press charges. Juvenile was issued citation.

Medical emergency. Dispatch advised that a 17 year old female had been hitting her head on the wall and was going in and out of consciousness. Patient taken to children’s hospital by MEMs for seizure prior to law enforcement arrival.

EMS Assist, Runaway, Runaway Juvenile, Self-Harm

QRTP – At approximately 1304 hours, officers responded to facility in reference to a runaway juvenile; Staff made contact with the juvenile, who then punched a staff member. Staff took the juvenile to the ground and officer placed cuffs on juvenile due to aggression. Juvenile attempted to self-harm with the cuffs, and while officer tried to reposition cuffs, juvenile began trying to choke and bite herself, and began banging her head on the ground. Juvenile continued aggression and self-harm attempts but finally complied after feeling faint because of the heat. MEMS responded and transported juvenile to ACH.

Battery, EMS Assist, Staff Attacked

Upon arrival, staff advised officer that after juvenile heard she was going to be put on special status that wouldn’t allow her to play games with her peers, juvenile attempted to leave the facility. Staff blocked the exit, and when juvenile tried to bite staff, a staff member grabbed juvenile to try and restrain her. Juvenile turned and punched the staff member in the face multiple times, attempted to bite her, and scratched her in the eye. MEMS transported juvenile to acute care. Officer cited juvenile and provided a court date.

QRTP – Upon arrival, staff stated juvenile was still agitated from an incident that happened last night. Staff attempted to deescalate juvenile but were unsuccessful. After trying to fight staff, juvenile was restrained until officer arrived. MEMS transported juvenile to ACH.

QRTP – At approximately 1820 hours, officers responded to facility in reference to a runaway juvenile and assisted staff in searching the area. Officers located juvenile at approximately 1927 hours; Juvenile was placed in handcuffs due to resisting officers and not cooperating. MEMS transported juvenile to ACH at facility’s request (no injuries).

Upon arrival, staff advised officer that he was attempting to calm down an angry student when he was struck on the side of his face with a closed fist. Staff advised facility has made arrangements to have juvenile transported to acute facility and requested officer stand by. Facility did not wish to press charges. MEMS transported juvenile.

EMS Assist, Juvenile Complaint, Suicide Attempt

QRTP – Upon arrival, juvenile told officer that some of the other kids were bullying her. Juvenile stated she told on her roommate for having a vape and pliers, and that her roommate was telling other kids that juvenile was a snitch and cuts herself. Juvenile had bandages on both arms and was seen by the nurse. When asked if she notified school or facility staff about this, she stated they did not do anything. Staff reported juvenile and her roommate were separated. Juvenile was transported to ACH by MEMS.

QRTP – Staff advised officers that juvenile attended her high school prom and was supposed to be picked up at 2300 hours. Juvenile was not answering her cell phone and had not returned to the facility. Officer completed Missing Persons form.

Buildings and Grounds, Visit Compliance Report

Licensing Specialist: Kendra Rice.
Facility visited from 12:30pm – 1:45pm. Census: 40. Walkthrough completed of Sturgis, Rose, Mabee, and Chestnut houses. Areas observed: milieu, bedrooms, bathrooms, and day areas. Clients’ rights and evacuation plans were not visible at Chestnut and Mabee houses; Staff reported that when maintenance touched up the paint, neither were put back in place. Staff requested those to be placed back on the walls.

Incident Report for placement client in our PRTF program and resides in Mabee House Incident Report date/time: 04/04/2023 9:30am Location of Incident: Siebert School. Incident Description: Pt is alleged offender for report put in that she forcefully touched a peer twice on one occasion and once on another occasion during the weekend and on Friday during tornado. The report was done via AR mandate reporter portal. The state police accepted the report. Licensing Specialist spoke with [REDACTED] (Unit Manager for Mabee House). [REDACTED] informed Licensing Specialist that there is no camera footage of these reported incidents. The allegation of residentbeing touched at the house happened on 3/31/2023 when the residents had to take shelter because of thetornado. The allegation of being touched while on an outing, there are no cameras on the vans. Licensing Specialist: Kendra Rice.

Combative and Suicidal Youth, EMS Assist, Juvenile, Suicide Threat

Upon officer’s arrival, staff stated that juvenile had become irate and began to destroy items. Staff restrained juvenile and placed her inside the seclusion room, and stated juvenile was suicidal and should be sent to the hospital. MEMS arrived and transported juvenile to the hospital.

Notice of Incident, Self-Harm

Client from in our PRTF program and resides in Sturgis House Incident Report date/time: 04/02/2023 2:10pm Location of Incident: Outdoors off Youth Home campus Incident Description: Self Injury(Not Suicidal), Medical Emergency (Trip to ER/Urgent Care). We arrived at our location for community outing and staff noticed there was a lot of people at the park. We informed the PTs that staff will assess the situation before we exit the van. PT begin to tell staff what he thought was best for us so. PT would not listen to staff information and immediately became visibly upset and verbally aggressive. I used CPI de-escalation procedures and PT did not calm down. PT wanted to leave but his peers were already engaged in activities. PT became more aggressive and threaten to harm staff. Pt then begin to punch the panel on the inside of the van and visible harmed his right hand. PT walked off and I continued to de-escalate and get him calm. After more conversation he finally calmed down and got in the van so that we could get him medical attention. The shift was informed of the situation and prepped the nurse for our arrival back to the house. Nursing assessment date/time: 04/02/2023 2:15pm: Patient presents at Sturgis outside main entry door with staff sitting on bench chair. Patient reported by staff to have punched the van dashboard while coming back from outing event. Pt reports pain 8/10 to right hand at Digit 5 and 4 knuckle area. Abrasions, redness, and edema noted at metacarpophalangeal joints at 4 and 5. Patient able to slightly perform some range of movement with pain noted. Cap refill is brisk at distal fingers of right hand. Orders received for transport to ACH for evaluation. 2nd Nursing assessment date/time: 04/02/2023 6:10pm: Patient returned from ACH on 4/2/23 at 4:45pm and no FX on xray. Patient is cooperative and calm.

Patient was having breathing problem and rapid breathing. Patient presents in his bed supine position upon assessment with rapid breathes. Patient endorses tightness in chest and throat. Patient was disoriented upon assessment to age and city that they live. On call was notified and ordered to have the patient transferred to ACH for evaluation.

Facility visited from 9am – 11:30am. Census: 41. Licensing attended CPI Training at facility to ensure staff are being trained according to the facility’s policy on physical restraints, and to ensure the policy complies with minimum licensing standards.

Technical Assistance, Visit Compliance Report

Licensing Specialist: Kendra Rice.
Review of the agency’s current mandated reporter policy determined it does not comply with A.C.A. 12-18-402 nor the Minimum Licensing Standards. Please ensure facility policy is updated to reflect the need of all staff to report directly to the child abuse hotline without delay. The updated policy shall be submitted to the Licensing Unit by 04/12/23 and all applicable staff shall be retrained on the updated policy within 2 weeks of the enactment of the updated policy.
TA –
- 110.9 – Any owner/operator, employee, foster parent, or volunteer in a child welfare agency shall immediately notify the Child Abuse Hotline if he or she has reasonable cause to suspect that a child has been subjected to child maltreatment, died as a result of child maltreatment or if they observe a child being subjected to conditions or circumstances that would reasonably result in child maltreatment.
- 102.3.h -Child Maltreatment/Mandated Reporter policy; according to Arkansas law, including, procedures to ensure that alleged, suspected, or witnessed incidents of maltreatment are reported to the Child Maltreatment Hotline, and documented as required by these or other applicable rule or laws.

Notice of Incident, Self-Harm, Visit Compliance Report

Client demanding to go out the front door. Client was encouraged to calm down. Client feeding off peer limit peer. Client began kicking the front door and hitting the wall with her right hand. Client then ran to restricted area and personal restraint was initiated. Encouraged client to relax, refused and continued to struggle with staff. Client was able to calm down and was released from restraint. Encouraged client to take good care of her hands because she will have them for life. Right hand 3-5th digit knuckles noted to be bruised and swollen. Limited ROM with mild/moderate pain, complains of numbness to 5th digit, capillary refill >3 seconds. Client not stable enough to go to urgent care with single staff supervision. Client shared that she wanted to kill herself. Encouraged client to make good choices, to give her future self a chance. Client voiced suicidal ideation once again, and the call was made to transfer to acute. Will continue to monitor. Client left for ACH ER, possible transfer to acute from there. Notice of Incident, Self-Injury, Suicide Attempt, Visit Compliance Report

Client placed in unlocked seclusion at this time as she attempted to force her way out this area without permission and de-escalation. Staff used their bodies to prevent her from leaving the seclusion room while providing verbal support. During this time client is observed to strike the wall of the seclusion room door with her fist. Client observed to say “I don’t fucking care.” and “I don’t want to fucking be here.” 4:13pm Unlocked Seclusion End: Client was able to force her way out of the seclusion room, and she agreed to sit with staff and talk. Unlocked Seclusion ended at this time. Patient Debriefing date/time: 03/14/2023 4:20pm: Client was given an option of talking to staff are going back to seclusion. Client chose to come out and talk with staff. Client was angry but willing to accept her consequences. Client said that she was not angry at staff anymore. Nursing Assessment date/time: 03/14/2023 4:15pm: Client reportedly in personal restraint. Upon my arrival, client was in seclusion room struggling with staff. Client agreed to sit and process with staff. Client began to calm down as she was processing with staff. Complains of right hand injury, 3rd-5th digits knuckles noted to be swollen, bruised and reddened. Client struck the wall with her right hand. Denies numbness/tingling, has full/active ROM, capillary refill <3 seconds. Ice pack refused, PRN OTC pain relief refused. Client denies other pain/injury from this incident. Client has no other obvious injury from this incident. Client was offered icepack and PRN medications again, refused. Encouraged client to use medication/icepack for pain/swelling. No other needs identified now, will continue to monitor. 2nd Nursing Assessment date/time: 03/14/2023 4:16pm: Client refused PRN OTC medication and icepack. Will continue to monitor. Guardian was notified on 03/14/23 at 4:20pm and 4:40pm. Was able to get a hold of her mother on the 2nd call. first call voicemail left. Licensing Specialist: Kendra Rice.

Notice of Incident, Self-Injury, Suicide Attempt

Suicide Attempt, Self-inflicted Injury
- Client was upset about “freeze” status and charged into the milieu hallway, then kicked the front door open. Client was placed in a restraint and then locked seclusion due to continued aggression. While in seclusion, she punched the wall and obtained a pen that fell off of staff’s lanyard during restraint. Staff entered and placed client in a restraint to retrieve the pen. Locked seclusion resumed and client removed her sweatshirt and attempted to tie it around her neck. Staff retrieved the shirt, and client forced her way out of seclusion and agreed to sit with staff and talk. Nurse assessed client and noted R hand 3rd-5th digits knuckles were swollen, bruised, and reddened. Client refused ice pack and pain medications.
- Licensing follow-up: On 03/16/23, Licensing reviewed video footage with no concerns noted. Staff reported client had her hand evaluated at the hospital and was transferred to acute placement.

Disturbance

Upon Officer’s arrival, staff stated a juvenile had assaulted 2 staff members and MEMS were needed. Staff 1 stated that after juvenile became angry, he was placed in a restraint and the seclusion room. Staff 1 stated that while in the seclusion room, he attempted to restrain juvenile by placing him against the wall and facing away from him, during which the juvenile threw his head backwards, hitting Staff 1 in the head. Staff 2 confirmed the same incident told by Staff 1, and stated that during the restraint, the juvenile swung his arm around and pulled on Staff 2s ear, pulling the earring out of his ear lobe. Officer observed Staff 1 had a large knot above his left eye and Staff 2 had a small laceration to his left ear. The facility requested the juvenile be transported to the hospital so he could be placed in acute care, and reiterated they did not wish juvenile to be charged and taken to jail. Juvenile was taken out of handcuffs and MEMS transported him to the hospital.

Officer responded to assist MEMS with a suicidal juvenile. Upon arrival, staff advised the juvenile was completely calm and compliant. While waiting on the ambulance, staff made the decision to transport the juvenile to the treatment facility in the company van; MEMS canceled.

Medical
- Client was transported to the ER due to inability to hold anything down and continuing to throw up and/or dry heave. Client reported he feels shaky on the inside and like he’s going to pass out.
- Licensing follow-up: Licensing was informed on 02/23/23 that client was still in the hospital and was discharged from the facility to his guardian. Client’s discharge was previously scheduled before his hospital admission. No licensing concerns noted.
