On 2/6/24, Resident was assessed by our Nurse Practitioner who found visible and palpable abnormalities in her neck. Resident was sent to Le Bonheur ER for medical evaluation. She had an ultrasound and Chest x-ray done and was diagnosed with Cervical Adenitis. She was returned to facility with antibiotics and ENT recommendation. Resident has a follow up appointment with ENT specialist today (2/8/24).

Census: 16. Licensing reviewed 2 personnel files and 2 children’s files. All files reviewed complied with the minimum licensing standards
Buildings & Grounds completed. The following areas were observed: bedrooms, bathrooms, butterfly room, seclusion rooms, day room, laundry room, and nurse’s station. The dining room was observed and there were residents eating lunch from the acute side. The unit was clean and all areas observed were organized. All furnishings were in good repair. MARs were reviewed. All initialed and up to date.

Client was seen at East Arkansas Family Medical Center yesterday afternoon due to running fever and complaining of stomach pain. It was recommended that client be taken to the hospital. Client was taken to LeBonheur Children’s Hospital and admitted due to kidney infection requiring IV antibiotics. Client is expected to return to Perimeter of West Memphis tomorrow.
2/5/2024 – Program Coordinator followed-up with facility to see if resident has returned. Resident has not returned to the facility and has been diagnosed with Pyelonephritis. Facility reported resident will be returning to the facility after treatment.

Officer made contact with staff in reference to a male patient attacking staff and causing a disturbance. The male subject left prior to officers arriving. The victims have already left the scene.

Notice of Incident, Peer Altercation
Resident 1 was in the cafeteria when she became irate over another resident accusing her of inserting herself in their conversation. Resident 1 threw a chair and almost hit Resident 2, who picked up the chair and threw it back at Resident 1. Resident 2 then laid down and kicked Resident 1 in the stomach then in the face. Staff separated the residents.
Resident 2 did not sustain any injuries. Resident 1 was assessed and was bleeding from her nose with a deformity of her nose noted. Resident 1 was sent to the ER for evaluation and was diagnosed with a nasal bone fracture. Resident returned to the facility with orders for neurological checks every 48 hours, no physical activities, bed rest, ice packs, and a referral to an ENT specialist. Residents were separated, placed on assault precautions and unit restriction.
Facility informed Licensing on 1/23/24 that Resident 1 will have to have surgery.

Officer Kimbrough responded to Perimeter Behavioral Health in reference to a fight. Officer made contact with Jeremy Woods who advised he heard a commotion and saw that two residents were in a fight over stolen property. Woods helped separate the two and grabbed the property. Resident 1 attempted to grab it back, then became irate and pushed Woods, grabbing him. Woods and witness, Bobby Crawford, attempted to put Resident 1 in a hold and Resident 1 punched Woods in the face. Then the two began fighting while Crawford and witness, Walter Sanders, attempted to separate the two of them. Resident 1 was taken to a holding room. Officers reviewed video footage and confirmed the statements of the staff and witnesses. Resident 1 was transported to CCJC.

Received call to make contact with facility in reference to a resident leaving the facility. Caller advised they picked the juvenile up.

Elopement, Notice of Incident
On 01/04/24 at 12:34PM, resident eloped from facility during fire alarm inspection (strobes and horns). Staff was unaware magnetic locks were also being tested during this time, as they normally would position themselves in front of exit doors during fire alarm drills. Resident eloped out of an unguarded exit door around the side of the building. Line of sight of resident was maintained. Resident returned herself willingly with staff at 12:40pm.
Actions Taken: Resident was medically assessed by Nurse Practitioner Roslyn Perry, APRN on 01/05/24 and reports she sustained no injuries and denies any pain or discomfort. Resident was placed on elopement precautions. West Memphis Police Department was called but was not needed as Resident returned prior to their arrival.

Officer Chase was dispatched to 600 North 7th in reference to a nurse being battered. Upon arrival, Hanna Brenner (Victim) stated that Arrestee (juvenile) was in a physical altercation with the smaller children. When Brenner tried to break up the altercation, arrestee pushed her to the ground, then broke through the secured door and jumped over the fence. Once Brenner caught up to arrestee, he yelled in her facing stating, “bitch I have choked your ass before”. Brenner advised that this was not the first time the arrestee has intentionally caused harm to her. Brenner advised that arrestee had previously choked her and banged her head against the wall. Arrestee has also caused injury to multiple individuals and has a habit of punching, spitting, and choking individuals. Brenner also advised that before the incident occurred that arrestee had threatened her multiple times to cause her harm.
Officer observed arrestee sitting in the corner advising the staff that they were ready to go to jail and that they did not like the nurses. Arrestee also admitted to trying to cause harm to Brenner a few days ago. Brenner had no bruises but advised that she had pain in her lower back. Arrestee was then placed into custody and transported to the Crittenden County Juvenile Detention Center where he was advised of charges. Intake Officer Murray advised to place a 24 hour hold on him and then release him back into the custody of Perimeter Behavioral.

Received a 911 call from this address from a subject who was laughing and advised someone was going to kill her. Caller then disconnected before advising further.
Received a second call from Kim in reference to a patient chasing people with scissors and attempting to stab others. Caller advised there will be staff at the front and back doors waiting to let officers in. Subject in custody. Send EMS to Juvenile for a subject needing to be checked out.

Notice of Incident, Peer Altercation
Resident1 was attempting to rest in her room and came out to ask the other residents to be quiet. At this time Resident1 and Resident2 began to have exchange of words and the Nurse was standing between the two residents trying to deescalate, when Resident2 ran up behind the nurse and shoved Resident1 into a bedroom where the two had a physical altercation. Additional staff responded immediately and separated the two residents.
Resident2 had no injuries. Resident1 sustained several scratches to her face, a knot on the back of her head, and complained of pain in her right hand. Actions Taken: injuries were assessed by RN, scratches were cleaned immediately and given ice pack. Resident1 was seen by our Nurse Practitioner that same day who ordered an in-house X-Ray of her right hand (X-Ray results normal). Scratches were ordered to be cleaned and ointment applied for 7 days and neurological checks for 48 hours. Resident1 did request to press charges against resident and a report was taken by West Memphis PD.

Officers arrived on scene and made contact with Perimeter Behavior Health staff stating that the arrestee was being disruptive and did not want to comply with the policies and procedures of the facility. Officers were advised by staff that Resident was no longer welcome to receive treatment and needed to be transported back to the Crittenden County Juvenile Detention Center. Officers advised that resident was being placed under arrest and being transported.
Resident became irate as officers escorted to the police cruiser. As resident was placed into the cruiser, he began to hit his head against the plexiglass divider. Resident began to bleed from forehead due to the constant banging against the divider. EMS was called to the scene to treat forehead. Resident repeatedly told officers and medical personnel that he did not want any medical treatment. Resident declined treatment or to be transported to the hospital by EMS but did allow medical personnel to irrigate the lacerations on forehead and bandage. Resident was transported to Crittenden County Juvenile Detention Center where he will be placed on a CIT hold for mental evaluation due to making statements that he wanted to die and wanting officers to shoot him.

Notice of Incident, Self-Harm
Resident became upset after having a verbal altercation with one of her peers. Resident went into the quiet room and flipped over two chairs and punched the wall. Staff placed resident in therapeutic hold for safety due to her self-injurious behaviors. Resident complained of pain on the right hand and the Nurse Practitioner ordered an in house X-Ray. The X-ray demonstrated a fracture. She was transported (12/16/23) to Baptist Crittenden to evaluate and treat.
Baptist diagnosed her with a closed boxer’s fracture of right hand, 5th metacarpal. A splint and ace bandage stabilized as well as pain prescriptions. Actions Taken: Resident returned to facility and was placed on Line Of Sight observation due to resident having an Ace bandage wrap. Resident is scheduled for an outside orthopedic appointment on 12/19/23.

Census: 18. Licensing Specialist completed buildings and grounds. The following areas were observed: grounds, dining room, dayroom, bathrooms, seclusion room, butterfly room, and nurse’s station. Ratio 1:1, a staff member and resident were walking through the area. Other residents were at school.
Grounds had parked cars and vans by kitchen area. Fallen leaves were on the ground. Licensing Specialist was escorted by Summer Berryhill, Director of Quality and Risk Mgmt.
Licensing observed the ratio (1:1) entering PRTF area,. All rooms observed were recently cleaned by housekeeping. At nurse’s station, Licensing observed staff charting and filing. Nurse Cindy escorted Licensing to the Observation Room were the medication cart is kept. Licensing reviewed the MARs. All initialed and up to date. Licensing was provide a copy of the 12/2023 activity calendar.
Licensing Specialist reviewed 3 children’s files. All files reviewed complied with minimum licensing standards.
Licensing reviewed 4 personnel files. T/A 105.15h (at least 3 positive personal references from non-relatives) – only had one reference verified. Facility was encouraged to request more references to verify. Other personnel files reviewed complied with minimum licensing standards.

Licensing Specialist spoke with Evelyn Alsup, CEO regarding a staff member mentioned in a maltreatment investigation. Ms. Alsup informed Licensing that staff member mentioned worked only on the acute side and not with PRTF residents. Licensing informed Alsup that she would follow-up at a later date for documentation.

On 11/26/23 Staff1 was approaching Resident1 in a threatening manner when another staff member intercepted. Simultaneously another resident (Resident2) ran and began hitting Staff1. A third staff member attempts to separate Resident2 from Staff1. Staff1 is seen reaching over the staff members attempting to strike Resident2′ s head and appears to have pulled her hair. Unable to determine if the strikes made contact. Resident did not sustain any physical injuries. Staff1 was removed from the facility immediately following this incident and termed the following day on 11/27/23. *This incident was reviewed in greater detail via camera footage 12/7/23. Prior to this date it was not discovered that Staff1 was attempting to strike Resident2. Report was made and this incident is currently under investigation.

Licensing spoke with Ms. Berryhill, Director of Quality and Risk Mgmt. Berryhill reported that camera footage was observed on 11/27/23 and staff member involved was terminated due to behavior. Keys were taken and she was removed from the facility. On 12/7/23, the Youth Coordinator reported that the staff member involved in this complaint might have hit the resident. Berryhill reviewed the camera footage again from different angles.
Facility will be cited for 109.1g. Staff member was observed charging at a resident in an aggressive manner. 905.4g, Staff member was observed trying to reach over other staff to grab resident.
CHRIS indicated case was found True. Complaint has been Founded by Licensing. Berryhill informed Licensing that staff has been retrained on mandatory reporting.

Accidental Injury, Notice of Incident
Resident was playing volleyball in the gym with a large exercise ball when she collided with another resident bouncing her back. Resident hurt her wrist and an x-ray was taken to assess injuries. X-ray results showed artifact but no fracture. Our Nurse practitioner completed a physical exam and the resident had full ROM and denies any pain or discomfort as of 11/20/23.
