Complaint: Patient drank toilet water with chemicals while not being supervised by anyone. Patient sleeping in boats on floors in main areas by nurses station. Ask for camera footage if they are working but they are. Patient and staff fighting and cellphone is used in front of patient and not enough rooms for patient to be placed cause patients just placed on top of each other. Patient attacks other patients all the time and nothing can be done about it.

Program Coordinator visited facility to discuss complaint. A walkthrough of the PRTF unit was completed to include all resident bedrooms/bathrooms and the milieu area. Areas viewed were very clean and orderly. Footage of the overnight shift on the PRTF showed no residents sleeping in facility “boats” outside their bedrooms.
- Facility cited 911.15: Bedroom B133 was found to have a pair of sweatpants with strings in them. They were immediately removed from the room.
- T/A provided (Reg 907.6): Review of 30-minute visual checks was unable to occur due to camera times jumping in 20-30 minute increments. Licensing requested facility to ensure all night shift staff are aware and educated on the visual check policy and minimum licensing requirements.
Facility reports a resident did allegedly drink toilet water containing chemicals on the acute side of the facility. They were taken to the hospital for observation, but it is unknown if they actually drank the toilet water.
Facility stated that very few altercations or issues have occurred on the PRTF side of the facility, and they provided an incident log for the past 30 days to the Program Coordinator. Program Coordinator discussed with the facility that incidents involving need for x-ray should be reported in ELS moving forward.
