Location: Little Rock, AR

Population Served: Adolescents 12-17 years of age in need of mental health services on a 24 hour a day basis.

# of residents per unit: 12 Residents per room: 1-2 Capacity: 54

Contact with family (Calls and visit schedule): Kids have daily contact with their family either via zoom, face time calls, or phone calls. In the evening they are given time to call families and their families are welcome to call anytime during the day. Outside of Covid-19 Restrictions families can visit on the weekends and once during the week.

  • Licensing (Injury Follow-up)
    • Client told staff he took off the cast to his left upper extremity that was placed on 8/25/22, due to it being loose. RN assessed client and noted finger to be bruised and swollen with limited ROM. Client was sent to the ER for replacement of cast.
    • Licensing follow-up: Specialist was informed by the facility that client has a follow-up appointment with Ortho Arkansas to determine if client will remain with a splint or get another cast.

  • Licensing (Suicide Attempt)
    • Client pushed peer after he made a comment about client skipping in line, and client attempted to enter staff’s office. Client was restrained due to aggression and was placed in seclusion. Client banged on the walls and removed his jacket and began choking himself; Staff intervened. Client then began to have a seizure in seclusion and when nurse arrived, client became aggressive. Once client calmed, he allowed the nurse to assess him. Once in his room, client tried to choke himself with a pillowcase; Staff intervened. After incident, client complained of pain to his arm and the side of his face and was given Tylenol. Client was placed on eyeball, front of house, and RSPA.
    • Licensing follow-up: Client was placed on eyeball (constant supervision).

  • Licensing (Medical)
    • Client had 3 episodes of vomiting around 7:10pm and was instructed to use a cool towel and lay down. Around 8pm, client went to the nursing station to receive medications. After taking medications, client fell out of a chair and proceeded to have 7 seizures. During this, 911 was called and client was taken to the ER via ambulance.
    • Licensing follow-up: Specialist requested update on client. Client returned to facility on 8/23/22 at 12:45am, and nursing note stated client was in good spirits and had no further incidents.

Officer was dispatched to assist MEMS with a psych patient they will be transporting. While in-route, Dispatch advised to disregard this call.

Notice of Incident, Suicidal, Threatening Behavior

  • Suicidal, Threatening Behavior
    • Client was made to leave group therapy for cursing at a peer and was informed privileges would be revoked. Client then slammed their head against a window and attempted to attack peer. A restraint was initiated, and once released, client went into room and shut the door. Staff explained the door needed to be open, and client then shut the door again. Staff opened the door to find client using a pencil to stab their wrist. Another restraint was initiated. Client later stated that given the opportunity, client would kill peer and then take their own life. Client placed on eyeball. No follow-up details.

Officer responded to facility in reference to suicide threats. Upon arrival, juvenile advised he was having suicidal thoughts and was stressed out from the facility and wanted to go to the hospital. Juvenile was involved in a physical dispute on 08/09/22. MEMS transported juvenile to Pinnacle Pointe Hospital.

  • Threatening Behavior, Assault
    • Client was personally restrained after attempting to attack a peer. During the restraint, client punched a staff member in the face, and 2 staff members hurt their thumb and forearm. The client was transported to Pulaski County Juvenile Detention.
    • Licensing follow-up on 8/11/22: Email received states that client was discharged from facility and that guardian had signed all discharge paperwork.

QRTP – Staff advised that juvenile was mad that he was back at facility and had seemed off all day. MEMS responded and transported juvenile to Rivendell Behavioral Health.

Staff stated while several staff were trying to break up a fight between 2 juveniles, juvenile struck staff member in the face and forearm intentionally. Juvenile was issued a court date, and staff members signed witness statements.

QRTP – Staff stated before she arrived at facility, juvenile had ran away and hit someone’s motorcycle with a stick (no damage). Staff stated juveniles then called her a snitch because she informed other staff what juvenile did. Juveniles (3) then began fighting staff and threatened to kill staff. Juvenile also attempted to grab Officer’s firearm and kicked Officer in the legs. Juvenile was transported to juvenile detention.

Officer was dispatched to take a report. Caller stated that on 7/11/22, juvenile attacked a child unprovoked and the child suffered minor injuries. Caller stated that on 8/1/22, juvenile attacked a staff member, destroyed a room, and made homicidal threats toward staff with a shank.

Complaint Visit, Self Report Visit, Visit Compliance Report

  • Licensing Specialist: Clayton DeBoer.
  • Facility visited from 9am – 1pm and viewed video footage of multiple incidents (7/27/22, 7/29/22, 8/1/22, 8/2/22. Facility made hotline call 8/2/22 for alleged maltreatment. For another incident, the nurse reported scratch client sustained from hold was reasonable for a hold.
  • Regs out of compliance –
    • 9.907.2 – Child caring staff shall be responsible for providing the level of supervision, care, and treatment necessary to ensure the safety and well-being of each child at the facility, taking into account the child’s age, individual differences and abilities, surrounding circumstances, hazards and risks. (8/2/22 incident)

    • At 1:09pm, client began threatening staff after being redirected, and refused to stay in room during quiet time. When client saw staff restrain a peer, client ran into the peer’s room and began to hit and kick staff. Client was restrained. At 1:50pm, client began self-harming by using fingernails to make wrist bleed. Client was restrained, during which client fought staff and tried to bang head on floor. At 2:20pm, a chemical restraint was administered.
    • Licensing follow-up on 8/3/22: Facility visited, and video reviewed. No inappropriate holds nor licensing concerns observed regarding this incident. Youth Home made this hotline call 8/2/22 for alleged maltreatment.

    • On 8/1/22, client was placed in a restraint due to self-harming and was placed on Eyeball (suicide watch). On 8/2/22, client was observed by staff reading a book, and when staff went to attend to other residents, a peer alerted staff that client was strangling themself with a shirt. Staff initiated a restraint, and client was transported via MEMS for acute placement.
    • Licensing follow-up on 8/3/22: Facility visited, and video reviewed. Staff reported that on 8/1/22 while off camera, client wrapped a shirt around their neck.

  • Suicidal, Threatening Behavior
    • Client was upset they could not be relieved of precautions and pulled a reflective metal mirror off the wall in their room. Client then attempted to obtain screws from the mirror, and staff intervened and initiated a restraint. Eyeball was added to other precautions (Run Risk, Supportive Status, House Restriction). Later that day, client became upset due to Eyeball status and barricaded a bathroom door. Staff forced the door open and initiated a restraint. Client was transported to acute placement via MEMS.
    • Licensing follow-up: Client estimated to discharge from acute placement on 8/4/22. Facility visited on 8/3/22 and video reviewed from 7/29/22. No visible nor audible licensing concerns observed in the video regarding this incident.

  • Suicidal, Threatening Behavior, Elopement
    • During Rec Group, client became anxious and asked to go back inside. On the way back, client became agitated and took off running. Staff followed, and client was placed in a personal restraint on the side of the road. After struggling with staff, a facility van arrived and transported client back to facility. Client stated they wanted to die. Upon entering facility, client was released from the restraint. Client went into room to change clothes and began hitting their head on the wall and hitting the door. Client was placed in another restraint until they calmed down. Client denied pain or injury and declined medication both during and after the restraint. Client placed on house restriction and run risk.
    • Licensing follow-up on 8/3/22: Facility visited, and video reviewed from 7/27/22, after elopement/restraint incident. Banging heard in audio at which point staff enter client’s room and initiate hold off camera. No licensing concerns visible or audible in video regarding incident.

Buildings and Grounds, Visit Compliance Report

  • Licensing Specialist: Clayton DeBoer.
  • Buildings & Grounds conducted from 9:30am – 11:00am for Rose and Mabee House. Census: 41. Outdoor recreation area observed. MARs checked for 5 clients.
  • TA –
    • R901.3 – When asked, a copy of client rights could not be provided. Staff obtained a copy of client rights to post in Mabee House before conclusion of today’s visit.
  • Regs not correctable –
    • R911.15.f – One bedroom had jacket in the closet with tie string in-seamed at least 3ft long. Staff immediately removed jacket and will remove string before returning jacket to room (Rose House).

QRTP – Staff stated juvenile was attempting to harm himself and possibly commit suicide. Juvenile was assessed by MEMS and transported to hospital at the request of facility.

  • Self-harm, Threat to Staff
    • Client became upset after receiving a consequence and began self-harming by picking at healing scabs on arms. After staff attempted to talk with client, client began hitting the wall. Client was restrained, during which client banged their head on the floor and head butted, punched, and bit staff. Client was offered oral medication but refused, stating they wanted a shot. Nurse obtained order for medication.
    • Licensing followed up on 7/28/22 via email (redacted).

Staff stated that after juvenile received consequence, he went to his room and began punching the wall until his fist became bloody. When staff went to assist, staff stated juvenile became violent and struck both staff several times, and bit one staff on the forearm. Juvenile was placed in seclusion and administered sedation medication. Juvenile issued court date and charged with 2 counts of Batttery in the second degree.

QRTP – Staff stated that juvenile ran away but returned a short time later. When staff asked juvenile to move to another couch once in facility, juvenile became irate and staff initiated a restraint. While staff were trying to gain control of juvenile, juvenile kicked a staff in the leg, spat at 2 staff, struck a staff in the back, and scratched a staff’s arm. MEMS arrived on scene and transported juvenile to ER.

  • ER Visit (Self-inflicted Injury)
    • Client punched the floor and complained of pain. Client was sent to ER and returned with a splint applied to left wrist (metal pieces removed for safety). Nurse assessed and Ibuprofen provided.
    • Licensing follow-up on 7/5/22: Client has splint on left wrist.

    • Client complained of pain to right hand and was sent to Urgent Care to rule out fracture.
    • Licensing follow-up on 7/5/22: Client has a follow-up appointment at Urgent Care on 7/5/22.

  • ER Visit (Accidental Injury)
    • Client reported they tripped in the gym while playing basketball. Nurse assessment: Moderate amount of bleeding present and a fairly deep laceration to chin. The area was cleansed, and client was sent to Urgent Care.
    • Licensing follow-up on 6/29/22: Client returned from Urgent Care with 4 stitches placed in chin. Instructions: Apply antibiotic ointment to wound and clean twice daily, return in 5 days to have stitches removed.

ER Visit, Notice of Incident, Self-Injury

  • ER Visit (Self-inflicted Injury)
    • Client reported they hurt their right hand while punching pillow up against the wall to release aggression. The nurse noted redness to knuckles and skin abrasion. Client was reassessed later in the day and sent to the ER. After returning from the ER, it was noted that the client had removed splint from their hand.
    • Licensing follow-up on 6/27/22: Per Nursing Note, client was diagnosed with hand injury and instructed to take Ibuprofen or Tylenol as needed for pain. Client received a splint to wear for the next 3-5 days. It was noted “Patient currently do not have splint on and no pain meds requested.”
