      • Failed to ensure there was an active order for use of restraint and/or seclusion procedure for client.
  • Plan of Correction: The Director of Nursing reviewed resident records and facility policies and provided training to all nurses. Random audits are also completed.

Stolen Electronics

Caller advised of stolen iPhone 11, and that the nurse on shift at the time that it was stolen stated that video footage was not viewable due to it being live feed. Caller stated she didn’t know if nurse was telling the truth or didn’t want to bother helping her.

  • Specialist and Assistant Director conducted unannounced monitoring visit, viewing all areas of facility. After completion of walkthrough, debriefing was held with CEO. Several residents were not wearing face masks during this time of pandemic.
