Battery, Resisting Arrest
On June 30. 2023 at approximately 7:26 pm, I, Officer J. Clark #275, made scene at 2050 East Broadway Avenue (Big stareast) to assist other officers with a shooting investigation. While on scene, I made contact with arrestee. While interacting with [REDACTED] I observed him to make several furtive movements towards his waistband and I noticed a slight bulge in that area. Officer D. Little #260 asked if he had a firearm on him and stated that he had multiple. The firearm was then removed from [REDACTED] and I attempted to detain him in handcuffs so that officers could safely check the rest of his person. Upon attempting to place in handcuffs, he stated that he was not going to comply and pulled away from me and started to assume a fighting stance. I then pushed [REDACTED] chest against of the front of a marked patrol unit and struggled with him, at which time he threw back his left elbow and struck me in the face causing my lip to bleed. Officer Z. Jones #253, Officer D. Little #290, Sgt. M. Mckee#229, and myself were then able to safely secure [REDACTED] in handcuffs. [REDACTED] was then transported to the West Memphis Police Department where he was interviewed by cid in relation to the shooting incident, and then I transported him to the Crittenden County Detention Center where he was advised of his court date and time.

Licensing Specialist completed buildings and grounds and observed the following areas: comfort room, laundry room, butterfly room, day area, bedrooms, bathrooms, and nurse’S station. The grounds were clean with no debris observed as Licensing Specialist walked to the building. The grounds leading to the gym were well kept. Licensing Specialist observed a picnic table and two (2) benches. Licensing Specialist was escorted by Summer Berryhill, Director of Quality and Risk Management. Residents were not present during the walkthrough. Ms. Berryhill informed Licensing Specialist that the residents were in the educational building. However, the butterfly room was occupied, ratio 1:1. Licensing Specialist observed the following bedrooms and bathrooms: b131, b132, and b133. All bedrooms were organized, and the bathrooms were recently cleaned. Licensing Specialist smelled the aroma of cleaning products. No plastic bags were observed on the unit. B131 had three (3) residents, b132 had two (2) residents, and b133 had three (3) residents. Licensing Specialist observed the following documentation on the walls: evacuation plan, gym time schedule, educational calendar, clients’ rights, grievance, and stress/Fear response system poster. This documentation was located by the milieu and day area. Licensing Specialist reviewed the mar’S on the residents listed below: all initialed and up to date. Licensing Specialist reviewed five (5) children files: [REDACTED]. All children’S files reviewed complied with the minimum licensing standards. Licensing Specialist: Kendra Rice.

Notice of Incident, Suicide Attempt
Client was on the unit upset attempting to find things to cause harm to herself. Resident ripped piece of plastic from trash bag and swallowed it. Resident appeared to be choking when our medical nurse practitioner intervened immediately.
Actions taken: resident was assessed by the nurse and vitals obtained, our nurse practitioner ordered the resident to be sent to the er for further evaluation. After visit summary states the resident had intentionally ingested foreign object and to continue regular diet and monitor bowel movement for object to be expelled. Resident has returned to our facility and been stepped up to inpatient acute unit for higher level of care and acuity.
Licensing narrative: Licensing Specialist informed resident obtained the piece of the garbage bag while on the unit and the issue has been corrected.

Officers made contact with a juvenile in the parking lot of the police department. Dispatch confirmed that juvenile was entered into NCIC/Nic as a runaway juvenile earlier that day. Juvenile was placed into custody and transported to Crittenden County Juvenile Detention Center where they were advised that their charges are disorderly conduct and obstruction of governmental operations for lying about their name to officers. At this time juvenile will be held until further notice or until an intake officer can instruct further.

Staff member reported that he was responding to assist other staff towards whom juvenile was being verbally and physically aggressive. When staff asked juvenile to exit the restroom, juvenile shoved and hit him in the face at least 3 times. Staff stated juvenile has completed the program and the facility has been attempting to contact DHS to get him, and they need a report made in order to force DHS to get juvenile.

Juvenile wheezing and vomiting, possible asthma attack. MEMS responded and transported juvenile to hospital.

Notice of Incident, Suicide Attempt
Resident tied two of her roommate’s shirts around her neck during hygiene time. Staff intervened and removed the shirts immediately. Resident was noted to have some redness on her neck but no further injuries were assessed by the RN on duty. Resident was alert and talking to staff, her vitals were within normal limits. Resident stated that she sees bloody demons that tell her to kill herself and also stated another resident told her to self-harm in the shower. Actions Taken: Physician was notified and ordered Resident to be placed on suicide precautions, level of observation was increased to 1:1 at all times, and placed in paper scrubs until able to contract for safety.
